On receiving this questionnaire form the Benefit Agency you should check the letter which is sent with it to find out which date the Benefit Agency want the form returned to.
Also check to see if the letter asks you to provide a MED4 form from your Doctor. The MED4 needs to be returned with your completed questionnaire.
Any written statements you can obtain from people such as physiotherapists, counsellors, epilepsy liaison nurse, that confirm your problems, would be useful.
Completing your Incapacity Benefit (IB50) form
To be awarded Incapacity Benefit you will need to reach a score of:
A minimum of 15 points if you have physical disabilities
A minimum of 10 points if you have a mental health problem
If you score at least 6 points in the mental health assessment these can be added together with the physical/sensory assessment: 9 points are added from the mental health assessment (whatever your actual score was) to the total score on your physical/sensory assessment to see if you meet the combined threshold of 15 points.

Be as realistic as you can about what you can and can't do, or what you have difficulty with when you complete the form. Use these guidance notes to help you, and if possible photocopy the form before you send it off and keep the copy in a safe place.
You will usually be asked to attend a medical after you have completed and returned your form.

Finally, some people are exempt from completing the Incapacity Benefit form. The rules are complex, an advisor may be able to explain this to you. If you are unsure, please ask us.
