Eligibility: Stroke survivours over the age of 18 who will benefit from inter-disciplinary rehabilitation.
Referral: Primary and Secondary Health Care, Social Service Professionals.
Description:
Aims to provide an integrated rehabilitation service for adults who have been affected by a stroke, or a stroke related illness, that will enable the individual to regain their optimum level of independence, within their own home or local families.

Aims:

  • To provide an inter-disciplinary team, who can be accessed by referrals from any source including Primary and Secondary Health Care, Social Services Professionals, Self, Family and/or Carer, Voluntary and Support Agencies.
  • To provide individual programmes of care designed to facilitate optimal recovery.
  • To provide an advisory and educational service that promotes the effectiveness of rehabilitation.
