Description:
Provides a short term support for people discharged from hospital to their home.
Eligibility: Adults admitted/discharged within the last month.
Referral: By telephone, Hospital staff, Social Worker, Intermediate Care, Self.
Cost: Please contact organisation for further information.

Aims:
To make contact with the client within 24hrs of discharge and can offer home visits and/or telephone support. Co-ordinators are Caroline Sissons and Kay Gormley. Please leave a message they will return your call.

Activities:
Have trained volunteers who visit/support and can help with:

• Social - check and chat.
• Check discharge and follow up concerns.
• Essential light household duties and shopping.
• Signpost to other services.
• Undertake talks to groups about their service.
