Description:
Tackling social exclusion and isolation.

Aims:
To support people who are either experiencing social exclusion or are at risk of being isolated through disability or long term illness, to become included in community life.

Activities:

• Through facilitating the set up of circles of support the Network have provided support for many hundreds of disabled people in the UK. All their work is based around the Values of Inclusion in all of their forms of support, making use of person centred planning, so ensuring that the focus person is in control of every aspect of the planning of their lives.
• They provide a range of services and training and can offer custom designed courses to fill all a person's training needs.
• The projects run throughout the UK, working directly with disabled people, people with learning difficulties and others who are at risk of exclusion or social isolation.
