Description:
Provides a short term support for people discharged from hospital to their home.
Eligibility: Must reside in/or be registered with a GP in DMBC area.
Referral: By telephone, Hospital staff, Social Worker, Intermediate Care, Self.
Cost: Service is free of charge.

Aims:

We aim to help people regain their confidence and independence.

Activities:
Have trained volunteers who visit/support and can help with:

• Social - check and chat.
• Check discharge and follow up concerns.
• Essential light household duties and shopping.
• Signpost to other services.
• Undertake talks to groups about their service.
